Hands-Only CPR vs Full CPR: What You Need to Know

full cpr training toronto

Many people hear about CPR and assume it means mouth-to-mouth breathing plus chest compressions. But guidelines have evolved, and in many situations, Hands-Only CPR (compression-only) can save lives, especially for bystanders. Here’s how the two compare, when each is appropriate, and how we teach both at Heart to Heart.

What Is Full CPR vs Hands-Only CPR

  • Full CPR
This includes chest compressions and rescue breaths. You are opening airway, giving breaths (mouth-to-mouth or using barrier), plus compressions. This is the standard version taught in many certification courses and especially important in situations involving children, infants, drowning, or when breathing problems have caused the cardiac arrest.
  • Hands-Only CPR
This is simply chest compressions, done hard and fast, without rescue breaths. The idea is to keep blood flowing through the heart to the brain until professional help arrives, especially when the bystander is not trained in or uncomfortable doing rescue breaths.

When to Use Each

Situation Use Full CPR Use Hands-Only CPR
Adult suddenly collapses in public, bystander present Either—if trained, full CPR; if not, hands-only is better than nothing Yes—hands-only is acceptable especially for those untrained
Child, infant, or someone who has had breathing issues / drowning Full CPR is strongly preferred Less ideal—hands-only may be better than doing nothing, but rescue breaths are more critical here
During high risk of disease transmission (e.g. infectious disease outbreak) Barriers masks and full PPE if available; if not, hands-only may reduce risk Hands-only may be more acceptable under these conditions

Key Steps & Best Practices for Effective CPR

  1. Rate of compressions: ~100-120 per minute.
  2. Depth: For adults, compress at least ~5 cm / about 2 inches; allow full recoil.
  3. Hand placement: Centre of chest, usually lower half of sternum.
  4. Minimize interruptions: keep compressions going until help takes over or signs of life.
  5. Use AED if available: early defibrillation is one of the most critical links in survival.

How We Train Both at Heart to Heart

  • We teach full CPR (with rescue breaths) on adults, children, infants, as required by Canadian Red Cross / WSIB guidelines.
  • We also make sure that every participant can do Hands-Only CPR — so even if someone isn’t confident with rescue breaths, they still leave knowing how to provide powerful compressions.
  • Our courses include practice on mannequins, guided feedback on compression rate, depth, hand placement, etc.
  • Scenario-based training: we simulate situations (e.g. collapse in public, child choking, etc.) so you can decide quickly whether full CPR or Hands-Only is appropriate.

Why Hands-Only CPR Is Important

  • Lowers barrier to action
Some bystanders feel squeamish or worry about disease transmission with rescue breathing. Hands-Only removes that hesitation.
  • Time critical
Brain damage begins within minutes of cardiac arrest. The faster compressions start, the better the chances, even if breaths are delayed or omitted temporarily.
  • Better outcomes in public cardiac arrests
Studies show that for witnessed adult sudden cardiac arrests, immediate compressions by bystanders double or triple survival chances. Even without rescue breaths, it’s far better than waiting.

Common Misconceptions

  • “If I don’t do rescue breaths, it’s useless” → false — compressions alone do a lot of good, especially early.
  • “I’ll break ribs / harm the person” → possibly you may, but ribs are bones and can heal; doing nothing often means far worse outcomes.
  • “Only medical professionals should do CPR” → no, many lives are saved by bystanders once trained.
